ERA requires that all athletes provide evidence of a sports physical or a doctor’s note citing the individual’s ability to be involved in competitive athletics. Previous physicals or a note from the doctor will be accepted as long as they were performed after the age of fourteen (14).
For both Middle School and High School programming, we require, at minimum, a doctor’s note. For High School programming, we highly recommend completing a sports physical.
Since each Physician has their own template, we do not provide a form to fill out. Please consult your athlete’s primary care physician about completing this item prior to season start.
